Job Description

This is a contract position where you will play a key role in supporting patients within the Emergency Department. You will assist individuals by identifying their needs, documenting essential health information, and connecting them with available resources to enhance their care experience.

Job Responsibility

  • Conduct one-on-one patient interactions within the Emergency Department to assess needs and provide support
  • Utilize the provided screening tools to evaluate social determinants of health and document findings accurately in the electronic medical record
  • Identify patients with positive screenings and coordinate referrals to social workers for further assistance
  • Provide patients with information about supportive services available through the Emergency Department
  • Access and navigate electronic medical record systems, including Altera, to document and retrieve necessary information
  • Collaborate with the Emergency Department team to ensure seamless patient care and effective communication
  • Maintain accurate and organized records of patient interactions and referrals
  • Work closely with the ED social worker to align schedules and enhance patient support during standard business hours
  • Ensure compliance with organizational policies and procedures while handling sensitive patient information securely

Requirements

  • Associate's Degree in a relevant field is required
  • Proficiency in electronic medical record systems, including Altera
  • Experience with Allscripts, Cerner Technologies, and other healthcare software systems
  • Strong ability to document and assess social determinants of health using screening tools
  • Familiarity with CRM systems, billing functions, and benefit authorizations
  • Excellent communication skills to interact effectively with patients and healthcare teams
  • Capability to manage inbound calls and provide accurate information
  • Detail-oriented with the ability to create and interpret charts and graphs
