Inpatient Psych Social Worker - LCSW position at Advocate Illinois Masonic Medical Center. Full-time role in Behavioral Health with a schedule of Sunday - Thursday, 830am - 430pm, no holidays. Responsible for discharge planning, clinical services, documentation, and functioning as part of a care management team.
Job Responsibility:
Discharge planning for hospital inpatients
Collaborate with other hospital disciplines, medical staff, and outpatient providers to develop plans for aftercare
Assess patients for discharge planning needs and identify resources
Organize and attend patient care conferences
Act as community liaison with outpatient providers
Establish and maintain positive working relationships with all referral sources
Coordinate and comply with legal requirements
Complete utilization review
Complete initial evaluation to identify patient needs
Develop and update discharge plan
Provide information regarding community resources for ongoing counseling services
Lead group therapy sessions as needed
Utilize crisis intervention techniques
Evaluate patient/family psychosocial factors
Provide appropriate clinical interventions
Timely and complete documentation
Functions as a member of the unit based care management team
Attendance at educational seminars as appropriate
Contributes to Continuous Process Improvement activities
Requirements:
MSW from accredited school of Social Work
Field experience within a hospital setting
Must recognize age specific needs and behaviors
Must adhere to NASW code of ethics and standards
LCSW licensure
Demonstrates postgraduate competence in social work clinical interventions and discharge planning
Ability to lift up to 35 pounds without assistance
