Job Description

Inpatient Psych Social Worker - LCSW position at Advocate Illinois Masonic Medical Center. Full-time role in Behavioral Health with a schedule of Sunday - Thursday, 830am - 430pm, no holidays. Responsible for discharge planning, clinical services, documentation, and functioning as part of a care management team.

Job Responsibility

  • Discharge planning for hospital inpatients
  • Collaborate with other hospital disciplines, medical staff, and outpatient providers to develop plans for aftercare
  • Assess patients for discharge planning needs and identify resources
  • Organize and attend patient care conferences
  • Act as community liaison with outpatient providers
  • Establish and maintain positive working relationships with all referral sources
  • Coordinate and comply with legal requirements
  • Complete utilization review
  • Complete initial evaluation to identify patient needs
  • Develop and update discharge plan
  • Provide information regarding community resources for ongoing counseling services
  • Lead group therapy sessions as needed
  • Utilize crisis intervention techniques
  • Evaluate patient/family psychosocial factors
  • Provide appropriate clinical interventions
  • Timely and complete documentation
  • Functions as a member of the unit based care management team
  • Attendance at educational seminars as appropriate

Requirements

  • MSW from accredited school of Social Work
  • Field experience within a hospital setting
  • Must recognize age specific needs and behaviors
  • Must adhere to NASW code of ethics and standards
  • LCSW licensure
  • Demonstrates postgraduate competence in social work clinical interventions and discharge planning
  • Ability to lift up to 35 pounds without assistance
