Requirements

MSW from accredited school of Social Work
Field experience within a hospital setting
Must recognize age specific needs and behaviors
Must adhere to NASW code of ethics and standards
LCSW licensure
Demonstrates postgraduate competence in social work clinical interventions and discharge planning
Ability to lift up to 35 pounds without assistance

Job Responsibility

Discharge planning for hospital inpatients
Collaborate with other hospital disciplines, medical staff, and outpatient providers to develop plans for aftercare
Assess patients for discharge planning needs and identify resources
Organize and attend patient care conferences to further identify each patient’s potential discharge needs
Act as community liaison with outpatient providers and collateral and connects patients to resources to ensure timely discharges
Establish and maintain positive working relationships with all referral sources
Coordinate and comply with legal requirements to ensure all patients are in appropriate legal standing
Complete utilization review for all patients in caseload
Complete initial evaluation to identify patient needs and place in medical record according to department policy
Develop and update discharge plan according to patient needs

2-3 years professional social work practice beyond the Master's degree required

Functions as Team Leader
Assists with case assignments throughout shift and oncoming shift
Assists in providing case consultation and supporting staff with case navigation and complex discharge planning
Assists in onboarding activities for staff in the service, including policies and procedures, as well as clinical teaching/orientation
Supports Senior Clinical Evaluators with to-do items for disposition cases, coaching Mental Health Assistants, and providing needed support and interventions on the floor regarding disposition patients
