Job Description

The Clinical Pharmacist is responsible for development, implementation,measuring outcomeand education of the antimicrobial stewardship program for the facility, analyzing and recommending action plan(s) for reported medication errors/adverse drug reactions, medication formulary management, order sets initiation/updates, evaluate clinical programs, preparation of clinical protocols/investigational studies, and other clinical activities requested by medical staff and pharmacy leadership. This will be accomplished through collaboration, coordination and communication with the Pharmacy Administration, Infection Preventionists, Pharmacists, Medical Staff and other healthcare workers. Ensures clinical effectiveness ofmedication (including antimicrobials) usage; decrease the emergence of resistance; education of prescribers and clinicians (including pharmacists, nurses and ancillary healthcare providers) in therapeutically efficacious and cost- effective therapy; decrease adverse reactions; improve patient care by shortening length of stay, preventing ADR; promote conversion of intravenous therapy to oral antibiotics; consolidate formulary; controlmedication costs.

Requirements

  • Bachelors of Science Pharmacy upon hire required
  • Doctor of Pharmacy upon hire required
  • Pharmacist: CA (PHARM:CA)
  • Two (2) years hospital experience or clinical pharmacy residency preferred
  • Experience in developing antimicrobial stewardship program or participating antimicrobial stewardship and implementing medication safety program
