Job Description:

Part-time board-certified nurse practitioner to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Engage with people and help them on their journey to better health through approximately 45-minute pre-scheduled visits.

Job Responsibility:

Conduct pre-scheduled in-home visits with health plan members
Provide at least 12 hours of availability weekly
Execute 35+ completed visits per month
Conduct Video Visits as needed
Complete all diagnostic and preventive service products as directed
Maintain training and complete required diagnostic and preventive services
Medication and medical history review
Physical evaluation
Additional diagnostic tests if ordered by health plan

Requirements:

Board-certified nurse practitioner
FNP or AGNP certification preferred
Active state licenses in coverage areas
Multi-state licenses preferred
Experience with In-Home Health Evaluations or similar format
Ability to travel locally and within licensed states (up to 25%)
Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds
Accommodate field observations and/or ridealong experiences

Nice to have:

Multi-state licenses
Experience with Signify Health In-Home Health Evaluations
FNP or AGNP certification
