Job Description:

Provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The visit, which lasts about 45 minutes, allows for time to connect one-on-one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests.

Job Responsibility:

  • Provide In-Home Health Evaluations
  • Engage with people in their homes
  • Conduct virtual visits as needed
  • Provide Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
  • Travel locally and within licensed states, as needed

Requirements:

  • Active, unrestricted license(s) in coverage area(s)
  • Multi-state licenses (preferred)
  • Able to cover the following primary locations: Seattle/ King County area
