Requirements

Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Maintain active state licenses in coverage areas
Maintain board certification (as applicable)
Maintain multi-state licenses (preferred)
Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies
Accommodate field observations and/or 'ridealong' experiences, as requested

Job Responsibility

Conduct pre-scheduled in-home visits with health plan members
Provide 40 hours of availability weekly
Execute a strong visit completion rate
Conduct Video Visits as needed
Maintain training and complete all diagnostic and preventive service (DPS) products as directed
