Job Description:

Signify Health, part of CVS Health®, is seeking a Clinician (Nurse Practitioner, Physician Assistant or Physician) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. This position involves conducting evaluations that include a medical history review, physical evaluation, and diagnostic tests as needed, while using an intuitive workflow system.

Job Responsibility:

Provide in-home health evaluations
Conduct virtual visits as needed
Medication and medical history review
Physical evaluation
Perform additional diagnostic tests when ordered by the health plan
Use intuitive clinical workflow on an iPad
Be flexible to travel locally and within licensed states

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification (required for NP/PA)
Multi-state licenses (preferred)
Able to cover the following primary locations: Wenatchee and surrounding area
