Job Description:

Signify Health, part of CVS Health®, is seeking a Clinician (Nurse Practitioner, Physician Assistant, or Physician) to provide In-Home Health Evaluations in individuals’ homes. Responsibilities include medication and medical history review, physical evaluations, and other diagnostic tests if ordered by individuals’ health plans. Clinicians will use provided tools and workflows and focus on patient engagement and bridging gaps in care.

Job Responsibility:

Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products
Bring your heart into every visit
Be part of our efforts to visit millions of people nationwide
Work with Signify Health as a clinician colleague

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification (required for NP/PA)
Multi-state licenses (preferred)
Able to cover the following primary locations: Riverside and surrounding area
