Job Description:

Signify Health, part of CVS Health®, is seeking a Clinician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. Flexible and personalized health evaluations ensure trusted relationships to make people healthier.

Job Responsibility:

Provide In-Home Health Evaluations
Medication and medical history review
Conduct physical evaluations
Perform additional ordered diagnostic tests
Use an iPad and intuitive clinical workflow
Conduct virtual visits
Provide Diagnostic & Preventive Services

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification (required for NP/PA)
Multi-state licenses (preferred)
Able to cover the following primary locations: Del Norte, Humboldt, Lassen, Modoc, Shasta, Siskiyou, Tehama, Trinity
