Requirements

Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Consistently engage with regular manager, team and clinical meetings
Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
Conduct Video Visits as needed
Maintain training and DPS-related supplies and diagnostic equipment
Maintain active state licenses in coverage areas
Maintain board certification (as applicable)
Maintain multi-state licenses (preferred)
Experience with Signify Health In-Home Health Evaluations or in-home visits (preferred)
Ability to sit, stand, bend, reach, push, pull, and lift 20 pounds of supplies

Job Responsibility

Conduct pre-scheduled in-home visits with health plan members
Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month
Execute a strong visit completion rate, consistently pacing 110+ completed visits per month
Conduct Video Visits as needed
Maintain diagnostic and preventive service-related supplies
Meet people in their homes for health evaluations
Review medication and medical history, conduct physical evaluations, and perform additional diagnostic tests

Requirements

maintain all required supplies and associated diagnostic equipment related to diagnostic and preventive service (DPS) offerings

maintain board certification (as applicable)
maintain multi-state licenses (preferred)
experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
