Job Description:

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner, Physician Assistant, or Physician) to conduct in-home health evaluations. Responsibilities include medication and medical history review, physical evaluations, and additional diagnostic tests as needed. The position does not require prescribing medication or altering current treatment plans. Clinicians engage with patients one-on-one to address health questions and receive support through a streamlined process.

Job Responsibility:

Bring your heart into every visit, joining a national network of purpose-driven clinicians
Be part of efforts to visit millions of people nationwide with the goal of connecting them to their unique care needs
Conduct virtual visits as needed
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification (NP/PA)
Multi-state licenses (preferred)
Able to cover the following primary locations: San Francisco and surrounding area
