Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Job Responsibility:

Conduct virtual visits as needed
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products
Bring your heart into every visit
Work with Signify Health as a clinician colleague
Be flexible to travel locally and within licensed states

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification
Multi-state licenses
2080 hours completed for all Advanced Practice Clinicians
Able to cover the following primary locations: Minneapolis and surrounding area or other areas of Minnesota depending on candidate location
