Job Description:

Signify Health, part of CVS Health®, is hiring full-time clinicians (NP, PA and Physicians) to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. The role involves conducting pre-scheduled in-home visits, engaging with health plan members, performing health evaluations, and coordinating with health plan providers.

Job Responsibility:

Conduct pre-scheduled in-home visits with health plan members
execute collaborating physician agreements
consistently engage with manager, team and clinical meetings
conduct video visits as needed
conduct medication reviews and physical evaluations during visits
maintain required supplies and diagnostic equipment
make referrals to health plans if necessary.

Requirements:

Active medical licenses (MD/DO) in coverage areas
trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
multi-state licenses (preferred)
experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies
flexibility to travel locally and within licensed states
maintain training and complete all diagnostic and preventive service (DPS) products as directed
coordinate collaborating physician agreement with contracted nurse practitioners or physician assistants.

Nice to have:

Multi-state licenses
experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format.
