Job Responsibility:

  • Support individuals to maintain tenancies and live independently
  • Create and deliver person-centred support plans
  • Provide practical advice and tenancy readiness training
  • Work in partnership with local services to prevent eviction
  • Travel regularly between two sites - one in Bradford, one in Leeds

Requirements:

  • A full UK driving licence and your own vehicle
  • Experience with vulnerable groups (e.g. homelessness, substance misuse, domestic abuse)
  • Empathy, resilience, and strong communication skills
  • Flexibility to occasionally work outside standard hours
  • Basic IT skills for case notes and reports
