Job Description:

The Housing Services Specialist is a fulltime position solely dedicated to the New Jersey market and is responsible for ensuring coordination of Housing Services with all other benefits and services provided to all Aetna Assure Premier Plus (HMO D-SNP) members. The Housing Services Specialist works in close partnership with the ABHNJ / Aetna Assure Premier Plus (HMO D-SNP) leaders, and all matrixed growth partners. Successful candidate must live in New Jersey or be willing to relocate.

Job Responsibility:

  • Assisting ABHNJ/Aetna Assure Premier Plus (HMO D-SNP) members in identifying, securing, and maintaining community-based housing
  • Make timely referrals to in-network Pre-tenancy and Tenancy Sustaining Services providers
  • Support the development of the housing components of the member’s MCO care plan, including setting measurable housing goals
  • Maintaining active listing of NJ housing resources including but not limited to HUD subsidized housing, all Department of Community Affairs (DCA), New Jersey Housing and Mortgage Finance Agency (NJ HMFA) housing program voucher programs, public housing authorities, realtors, and online housing locator resources
  • Maintain regular contact with housing providers to ensure service quality and address any issues
  • Monitor provider performance and member satisfaction, escalating concerns to the Housing Services Manager
  • Participate in quarterly reporting and quality improvement initiatives as required by state officials
  • Stay current on housing resources, eligibility criteria, and regulatory updates

Requirements:

  • At least three (3) years full-time experience in assisting vulnerable populations such as MLTSS, elderly, people with disabilities and homeless to secure accessible, affordable housing
  • Familiarity with NJ public and private housing resources and stakeholders, including but not limited to HUD subsidized housing, Department of Community Affairs (DCA), New Jersey Housing and Mortgage Finance Agency (NJ HMFA) housing program voucher programs, public housing authorities, realtors, and online housing locator resources
  • Strong analytical and communication skills (written and verbal)
  • Excellent organization and time management skills
  • Must possess reliable transportation and be willing and able to travel up to 20% of the time
  • Bachelor’s degree in social work, public health, human services, or a related field
  • equivalent experience may be considered
