Job Description

The Housing Placement & Stabilization Specialist supports individuals living in low-income or specialized housing by providing case management, housing stabilization services, and connections to community resources. This role focuses on helping clients maintain housing stability, meet financial obligations, and build long-term supports through structured outreach and follow-up.

Job Responsibility

  • Manage an assigned caseload of clients residing in low-income or supportive housing
  • Conduct stabilization visits to assess housing stability and immediate needs
  • Connect clients to community resources and supportive services
  • Support clients with budgeting and making timely rent payments
  • Coordinate with internal teams and external service providers to address barriers to housing retention
  • Maintain accurate documentation and case notes
  • Provide ongoing support to promote long-term housing stability

Requirements

  • Associate’s Degree in Human Services or High School Diploma with 3+ years of relevant experience
  • Minimum 6 months of experience working with individuals experiencing homelessness or similar populations
  • Prior case management experience (required)
  • Ability to manage an assigned caseload and conduct community- and home-based visits

  • Build and maintain opportunities by researching available units, developing landlord relationships, and supporting placement efforts as a housing navigator
  • Advocate for clients through communication and mediation with landlords, relatives, and other support networks to help resolve barriers to stable housing
  • Coordinate financial assistance related to rent, utilities, and other essential housing costs, including helping clients complete required applications and planning for ongoing affordability
  • Link clients with community-based services such as healthcare, public benefits, employment support, legal resources, transportation assistance, and educational services
  • Track client progress, make timely referrals and related programs, and work closely with internal staff and external partners to improve outcomes
