Job Responsibility

  • Conduct intake, screening, and assessments to identify housing barriers, needs, and preferences
  • Develop individualized housing/service plans in collaboration with Veterans and VASH case managers
  • Provide short-term (up to 6 months) case management to support transition into VASH services
  • Assist Veterans with housing search, placement, and use of Rapid Re-Housing to bridge delays
  • Coordinate rental assistance, move-in logistics (furniture, transportation), and community resource connections
  • Partner with VA Medical Centers, VASH teams, and community providers
  • attend case conferences and weekly calls
  • Maintain accurate documentation, meet reporting deadlines, and ensure program compliance

Requirements

  • Bachelor's degree in Human Services or related field
  • 1+ years of experience in case management, housing services, or placements
  • Experience working with homeless or at-risk populations, including exposure to mental health or substance use
  • Strong communication, documentation, and database/computer skills

Nice to have

Experience with Housing First, Harm Reduction, and Trauma-Informed Care approaches

  • Participate in Continuum of Care (CoC) meetings and similar housing coalitions, advocating and collaborating to expand housing opportunities for refugee populations, individuals with disabilities, and older adults
  • Secure and confirm safe, clean, and affordable housing for clients across multiple programs, including application, lease signing, and apartment set up
  • Assist clients in securing financial assistance, including housing deposits, moving costs, reasonable housing accommodations, and other necessary supports
  • Conduct individualized housing needs assessments and develop housing support plans tailored to each client
  • Provide comprehensive case management services, including maintaining accurate and timely client notes and documentation in accordance with program standards
