Job Responsibility

Serve as a critical liaison between physicians, the patient care team, and dispensing pharmacies to facilitate seamless communication and collaborative patient management
Coordinate and schedule patient follow-up appointments
Provide support for essential administrative processes, including assisting with prior authorizations, coordinating discharge prescription processing, and facilitating death certificate completion
Aid physicians in the preparation of official patient documents, such as FMLA, short-term disability, and return-to-work forms
Systematically track, organize, and maintain accurate records of all new patient encounters
Apply robust clinical judgment and the nursing process to advance patient health, well-being, and safety in strict accordance with the State Nurse Practice Act and care standards
Maintain personal accountability, act as a professional role model, and uphold the highest ethical, legal, and professional nursing standards
