Job Responsibility

  • Serve as the bridge between hospitals and hospice—facilitating seamless transitions of care
  • Provide education, guidance, and emotional support to patients and families making hospice decisions
  • Collaborate with physicians, case managers, and social workers to identify and coordinate patient referrals
  • Complete hospice admissions within the hospital setting and ensure documentation accuracy in both Blue Ridge Care and hospital EMRs
  • Conduct supportive visits for existing hospice patients admitted to hospitals and communicate updates to care teams
  • Advocate for patient comfort by initiating nursing interventions that relieve pain and other symptoms of life-limiting illness
  • Provide formal and informal education to healthcare professionals about hospice philosophy, benefits, and symptom management
  • Participate in rotating holiday and weekend coverage to ensure continuous, compassionate service delivery

Requirements

  • Current Registered Nurse (RN) license in Virginia
  • BSN preferred
  • Strong communication and collaboration skills with hospital and interdisciplinary teams
  • Proficiency in electronic medical records (EMR) and computer systems
  • Ability to travel throughout assigned hospital and community territory
  • A heart for patient advocacy, education, and compassionate service
