Job Description:

As part of a fast-paced multidisciplinary team, you will act as the vital link between acute hospital wards and community services. Your goal is to reduce 'delayed transfers of care' and ensure patients have the right support packages in place to recover safely.

Job Responsibility:

  • Conducting holistic assessments of patients' needs under the Care Act (2014) or Continuing Healthcare (CHC) frameworks
  • Liaising with doctors, therapists, families, and home-care providers to build robust discharge plans
  • Identifying barriers to discharge and implementing creative solutions to support 'Home First' initiatives
  • Completing Mental Capacity Assessments and Best Interest Decisions where required

Requirements:

  • Must be a Qualified Social Worker (registered with Social Work England) or a Registered Nurse (NMC)
  • Previous experience in a hospital setting, rapid response, or intermediate care is highly desirable
  • Strong communication skills and the ability to work under pressure in a high-intensity environment
  • Familiarity with Newcastle-based community resources and adult social care pathways is a plus
