Job Description:

The Hospice Registered Nurse is responsible for pain management, symptom control in the delivery of care to hospice patients, as established by the plan of care, the interdisciplinary team, attending physician and Hospice Medical Director.

Job Responsibility:

  • Pain management and symptom control for hospice patients
  • Ensures timely and adequate delivery of hospice services
  • Assesses total needs of patient/family during home visits
  • Coordinates total patient/family hospice care
  • Collaborates with interdisciplinary team on plan of care
  • Ensures continuity of care between all parties
  • Communicates needs, assessments, interventions, goals and outcomes
  • Provides education on hospice concept of care
  • Supervises delivery of care by Hospice Aides and Licensed Practical Nurses
  • Facilitates delivery of services to patients in long term care facilities
  • Participates in on-call rotation
  • Monitors patient appropriateness for hospice services
  • Maintains compliance with Medicare, Joint Commission, and agency standards
  • Supports community education of hospice
  • Dispenses medical supplies cost-effectively
  • Participates in quality improvement activities
  • Attends inservices and continuing education
  • Ensures accurate and current patient chart documentation
  • Maintains professional appearance
  • Maintains privacy and security practices

Requirements:

  • Current license as a Registered Nurse in the State of Alabama
  • Minimum of one (1) year medical/surgical experience
  • Hospice/Home Health experience preferred
  • Valid CPR and Drivers license
  • Compliance with all health-related requirements in relevant jurisdictions, including required vaccinations and testing
