As a Nurse Practitioner with CommonSpirit Health at Home, you’ll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient’s continuing eligibility for home care or hospice care. This role may involve providing services in various settings, including the patient’s home, a physician’s office, or an inpatient hospice facility. Job Responsibility:
Provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient’s continuing eligibility for home care or hospice care
Provide services in various settings, including the patient’s home, a physician’s office, or an inpatient hospice facility
Adhere to the Corporate Compliance Program, including confidentiality of HIPAA protected health information
Provide care following the plan of treatment and patient care plan to assist in the determination of hospice eligibility
Establish a care plan for continued needs based on the client assessment
Report changes in patient condition to appropriate personnel and the attending physician
Provide patient care education, counseling, medication advice, instructions, and follow-up to clients and other caregivers
Participate in coordination of services with other disciplines and community resources as needed
Evaluate and revise care plans/assessment sheets based on changes in patients and/or the environment
Follow the sign-in procedure and the policies and procedures for that particular institution when providing private duty or staffing in a facility
Present patient assessment information to the supervising physician
Perform appropriate emergency measures as deemed necessary
Assist with the development of educational programs in the hospice program for diverse age groups
Provide paraprofessional supervision as required by policy and regulations
Complete and submit all required documentation in a timely manner
Regulate and adjust medications as indicated
Demonstrate sensitivity to patient and/or family needs, customs or feelings
Participate in case conferences
Recognize emergencies and has knowledge of emergency procedures
Requirements:
Graduate of an accredited school of nursing
Currently licensed to practice in the state serviced, with a Master's in Nursing
At least 1 year of nursing experience
Must obtain and maintain current CPR Certification
Valid driver’s license and auto insurance coverage according to the CommonSpirit Health at Home policy
Compliant with appropriate state certification/registration and CommonSpirit Health at Home requirements as necessary
Nice to have:
One year of hospital nursing experience
