Hospice Nurse Practitioner NP PRN

• Performs the face to face encounter within 30 calendar days prior to start of the 3rd benefit period and all subsequent re-cert periods
• Gathers and documents clinical findings to determine continued hospice eligibility
• Shares the clinical findings with the hospice medical director
• Attests in writing that he/she performed the face to face encounter and that these findings were provided to the certifying physician
• Communicates with Clinical Director and RN Case Manager prior to visits as to pertinent details of patient's disease process and outcome of the actual face to face visit
• May make patient consults and visits as appropriate and as requested
• May attend interdisciplinary team meetings as appropriate
• Provides in service education as requested
• Maintains sound privacy and security practices and prevents privacy or security breaches
• If breach occurs, takes corrective action

• Current License as Nurse Practitioner in the State of Alabama
• Minimum of one (1) year medical/surgical experience
• Current, valid Alabama Driver's License
• Proof of automobile insurance coverage
• Valid CPR certification
• comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing

Hospice/Home Health experience preferred
