Requirements

Graduate of an accredited practical nurse or a vocational nursing program
Three years of nursing experience
Currently licensed as an LPN/LVN in the practicing state
Must possess current CPR Certification
Valid driver’s license with an automobile that is insured in accordance with state requirements
Understands the philosophy of hospice concept and the needs of the terminally ill

Job Responsibility

Provide direct patient care as defined in State Nurse Practice Act
Implement current nursing practice following a comprehensive assessment and the plan of care
Provide accurate and timely documentation of patient services to reflect the plan of care
Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group
Provide appropriate pain/symptom management
Evaluates the patient’s response to treatments/medications

Document customer interactions and resolutions accurately in our support ticketing system
Work with Support Leadership to recognize usage trends and opportunities for more efficient customer service
Collaborate cross-functionally with Product, National Implementations, and the Distributor team by logging feature enhancements, connecting with account managers, staying up-to-date on distributor offerings and industry trends, etc.
