Job Description:

Provide trauma-informed, flexible case management using Housing First principles. Support families, adults, and youth facing persistent homelessness. Focus on securing social housing, improving health, and capacity building.

Job Responsibility:

  • Provide trauma-informed, flexible case management using Housing First principles
  • Support families, adults, and youth facing persistent homelessness
  • Focus on securing social housing, improving health, and capacity building
  • Deliver comprehensive case management and tenancy support guided by Australian Housing First Principles to achieve long-term housing stability
  • Provide holistic, strength-based support and intensive, flexible assistance for young people, single adults, and families with complex needs and increased vulnerabilities (e.g., trauma, AOD)
  • Work effectively in a multidisciplinary team to conduct culturally safe care coordination, service navigation, and referral to mainstream and specialist services
  • Trauma Informed Care and Practice to support clients in obtaining and sustaining housing, using flexible brokerage to address their individual needs and goals
  • Actively participate in community networking and convene multi-disciplinary team meetings to ensure an integrated, effective support system for clients

Requirements:

  • Extensive experience providing holistic, strength-based, trauma-informed case management to vulnerable individuals and families with multiple and complex needs
  • Demonstrated knowledge of the contemporary homelessness context, Australian Housing First Principles, and the ability to apply this to determine and deliver effective, consumer-led housing solutions
  • A strong commitment to culturally sensitive practices, effective service delivery, and the capacity to build strong links within relevant community networks (especially in Gippsland)
  • Valid Victorian Driver’s Licence
  • Working with Children Check
  • Police Check
  • Minimum requirement of Diploma of Community Services or other appropriate tertiary qualification/s
