Join our team to provide case management for clients experiencing homelessness. This role involves both transitional support (holistic capacity building) and immediate crisis response.
Job Responsibility:
Deliver comprehensive case management and intensive, transitional support to young people, single adults, and families experiencing homelessness
Conduct holistic, strength-based assessments to develop tailored, goal-oriented plans focusing on long-term housing stability and independent living
Provide expert care coordination, service navigation, and referral to connect clients with mainstream and specialist services (health, mental health, AOD, etc.)
Offer targeted, flexible, and rapidly responsive support to clients with increased vulnerabilities and complex needs (e.g., trauma, multiple health issues)
Work collaboratively with other agencies, participate in community networking, and convene multi-disciplinary team meetings to ensure integrated client support
Requirements:
Proven expertise in holistic case management and care coordination for vulnerable people, including the ability to conduct strength-based assessments and develop tailored, goal-oriented plans for long-term housing stability
Demonstrated skill in supporting clients with complex needs (e.g., trauma, mental health, AOD) by providing targeted, flexible, and rapidly responsive support
Strong collaborative and networking abilities to effectively coordinate care across multiple agencies, convene multi-disciplinary teams, and ensure integrated support for clients
Valid Victorian Driver’s Licence

Minimum requirement of Diploma of Community Services or other relevant or appropriate tertiary qualifications
