Job Description
The Home Ventilation Team provides specialist care and support to children and young people who require long-term ventilation. The team works predominantly in the community but also reviews acutely unwell patients on the wards and supports consultant colleagues in clinic, delivering a service across the region. In this role, you will provide specialist nursing care to a varied group of patients across multiple settings, planning and evaluating care using appropriate clinical pathways.
Job Responsibility
Write individualised care plans that reflect each child's wider health needs and how these affect their ventilation requirements.
Deliver developmentally appropriate, specialist nursing care to a diverse group of patients across a range of settings, including inpatient, outpatient, day care, community and educational environments.
Plan and evaluate complex care using appropriate pathways, working collaboratively with the multidisciplinary team.
Run nurse-led clinics alongside consultants, providing telephone advice and carrying out home visits as part of the service.
Contribute to the development and implementation of policies, guidelines and standard operating procedures to support high-quality, consistent care.
Requirements
Current NMC registration
Relevant post registration qualification as required by speciality
Mentorship/supervisor training
Experience of providing clinical advice in relation to specialist area
Experience of planning provision of care for patients
Mentoring experience
Knowledge of clinical governance activities
Ability to dynamically risk assess clinical situations & respond accordingly
Effective communication skills
Ability to build relationships for collaboration with MDT and external agencies.
Ability to respect cultural, social & religious diversity within society
Ability to manage complex & challenging situations with empathy
Ability to adapt to different settings & patient needs.
