Job Description

Provides skilled professional nursing services to a case load of patients in their place of residence in accordance with the patient’s plan of care.

Job Responsibility

  • Assesses patients prior to provision of care
  • Provides ongoing assessment of the patient and patient’s equipment needs
  • Initiates timely discharges
  • Educates and instructs the patient, patient’s family, or staff as required
  • counsels patient and family in meeting patient’s needs and goals
  • Prepares timely, legible, and complete documentation of all care, interventions, and care coordination as provided by law, regulation, and established policy
  • returns documentation to the office/department according to established policy
  • Demonstrates an awareness of and sensitivity to patient/family rights
  • Applies safety principles as identified by established policy
  • Participates in the orientation and training of new staff
  • Participates in care coordination meetings
  • Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices
  • Ensures compliance with legal issues, including but not limited to patient confidentiality and risk management
  • ensures compliance with ACHC/HIT, federal, state, and local regulations
  • Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals
  • Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality
  • Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator
  • Performs other related duties as assigned by management

Requirements

  • Must possess a current Arizona RN license
  • Must possess a current BLS certification
  • Minimum of one year related experience or equivalent
  • Must possess a valid driver’s license and auto insurance
  • Proof of TB screening
  • Demonstrated competence in reacting to and handling emergencies
  • Strong charting/documentation skills
  • Ability to understand and adhere to established policies, procedures, and protocols
  • Commitment to excellence and high standards
  • Excellent written and oral communication skills
  • Strong organizational, problem-solving, and analytical skills
  • Ability to manage priorities and workflow
