Requirements

Minimum of six (6) months of recent work experience, preferably in-home care
Current licensure in your state of employment
Satisfactory references
Satisfactory completion of Agency-LPN/LVN Qualifying Exam
Proof of satisfactory completion of an approved pharmacology course (i.e., NAPNES) may be requested
Current CPR certification
Must have Driver's license, current auto insurance, or access to reliable transportation
Current TB, Physical, and Clean background check

Job Responsibility

Recognizes and reports symptoms that indicate changes in the condition of the patient to the Nursing Supervisor and/or Case Manager and/or physician as appropriate
Provides nursing care and services that are in accordance with Agency patient care and general medical policies
Assists the physician and/or Registered Nurse in the performance of specific procedures required by the patient and at an appropriate level for the LPN / LVN
Observes, record, and report to the appropriate professional the symptoms, reactions, and changes
Performs selected nursing procedures, including administration of medications and treatments prescribed by the physician
Preparation and care of patients receiving specialized treatments administered by the physician or the professional nurse
Assists with the rehabilitation of patients according to the medical care plan
Promotes cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences when appropriate
Administers medications, IVs (when allowed by state regulation), and treatments, observing aseptic techniques adequately

Requirements

Graduate from an accredited school of practical nursing
Licensed as a vocational/practical nurse in the state of practice
At least two (2) year clinical practical nursing experience
Ability to make home visits and provide nursing care in the home environment
Ability to complete necessary documentation appropriately, legibly and timely
Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
No weight restrictions
Must have reliable transportation, current driver's license and appropriate automobile insurance
or ability to take public transportation

Job Responsibility

Perform selected nursing procedures, commensurate with experience and education within the scope of LPN/LVN practice including but not limited to: tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, and phlebotomies
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN. Document the patient's file accordingly
Work with patient, family and health care team members effectively to coordinate patient care
Maintain a high degree of confidentiality at all times due to access to sensitive information
Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency
Follow all Medicare, Medicaid, and HIPAA regulations and requirements
