As a Home Health Scheduler you will play a pivotal role in coordinating and maintaining efficient schedules for our clinical staff, ensuring seamless delivery of care to our clients. This position requires excellent organizational skills, attention to detail, and the ability to manage multiple tasks in a dynamic environment.
Job Responsibility:
Develop and manage daily schedules for clinical staff, including RNs, LPNs, PTs, OTs, STs, home health aides, and social workers, aligning with patient care plans and staff availability
Coordinate patient visits based on individualized care plans and authorized services
Communicate schedule changes promptly to staff and clients, ensuring clarity and continuity of care
Maintain accurate records of staff assignments, patient visits, and any deviations from scheduled services
Utilizes system reports to ensure visits are scheduled with correct codes to ensure regulatory requirements are met (specifically therapy reassessment time frames)
Monitor compliance with agency policies, payer requirements, and regulatory guidelines, including CMS standards
Collaborate with clinical managers to address staffing needs, patient concerns, and service adjustments
Assist in tracking and reporting key performance indicators related to scheduling efficiency and patient satisfaction
Willing to learn other office functions to assist as needed and provide coverage as needed

Requirements:
Previous experience in computerized scheduling
Minimum of 2 years clerical experience
Knowledge of medical terminology preferred
Ability work in a fast-paced environment, prioritize tasks
Excellent communication skills

Able To Keep Confidential Information Regarding Patients, Team Members
Able To Withstand Crisis Situations
Has Skills To Provides Customer Service To Patients, Team Members And Visitors
Knowledge And Experience With Electronic Health Records

High school graduate or equivalent
