Full time Home Health RN Case Manager position serving North Chicago, Waukegan, Park City, Lake Forest, Highland Park areas. Day shift, Monday-Friday 8-5 schedule with weekend rotation, on-call schedule, and holiday rotation. Position includes patient care, quality improvement, patient/staff education, nursing process application, and professional leadership/collaboration.

Facilitates the patient and family’s right to receive quality, cost effective care
Demonstrates a commitment to meeting the learning needs of patients and families
Demonstrates the use of nursing process as a problem-solving model
Demonstrates respect and understanding for peers and other clinical disciplines and participates as an effective member of the patient care team
Requirements:
Graduate from an accredited school of nursing
One year of medical surgical experience
Has demonstrated proficiency for the requirements for NC I
Good communication and interpersonal skills
Good documentation skills
Computer skills
Good time management and organizational skills
Familiarity with patient care equipment
Current Illinois RN license
Current CPR
Ability to work weekends, holidays, and different shifts
Ability to take “on call” assignments
Ability to handle multiple tasks and complete work within short timeframes
Ability to transport objects, patients, and equipment
Ability to respond quickly to patient needs and work at fast pace
Ability to lift up to 35 pounds without assistance
Must be able to travel to all areas within the Advocate service area
Must be willing to work occasional overtime if needed
