Job Description

Full time Home Health RN Case Manager position serving North Chicago, Waukegan, Park City, Lake Forest, Highland Park areas. Day shift, Monday-Friday 8-5 schedule with weekend rotation, on-call schedule, and holiday rotation. Position includes patient care, quality improvement, patient/staff education, nursing process application, and professional leadership/collaboration.

Requirements

  • Graduate from an accredited school of nursing
  • One year of medical surgical experience
  • Has demonstrated proficiency for the requirements for NC I
  • Good communication and interpersonal skills
  • Good documentation skills
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license
  • Current CPR
  • Ability to work weekends, holidays, and different shifts
  • Ability to take “on call” assignments
  • Ability to handle multiple tasks and complete work within short timeframes
  • Ability to transport objects, patients, and equipment
  • Ability to respond quickly to patient needs and work at fast pace
  • Ability to lift up to 35 pounds without assistance
  • Must be able to travel to all areas within the Advocate service area
  • Must be willing to work occasional overtime if needed

  • Provides on-going assessment of patient response to treatments and teaching and applies interventions as appropriate. Updates and revises plan of care as indicated
  • Completes documentation including visit assessment, OASIS, 485 and verbal orders accurately and promptly and within the standards
  • Communicates with physicians to confirm and update patients' plan of care. Provides status updates and ensures ongoing communication with the physician and other members of the health care team
  • Facilitates the discharge planning process according to standards and utilizes resources to assure continuity of care after discharge. Participates in developing the discharge plan with the patient/family, physician, social worker, and other staff members as appropriate
  • Participates in chart audits and continuous quality improvement activities as requested. Identifies, develops and participates in process improvement opportunities that will enhance the quality of the services provided
  • Maintains and updates accurate clinical and patient records according to agency, state, and federal guidelines

  • Oversees the primary needs of the patient, including assessment of patient and plan of care, and implementation of prescribed interventions

  • Conducts thorough physical, psychosocial and environmental assessments of assigned patients and updates plan of care where appropriate and as a result of visit finding
