PRN position for a Home Health Registered RN in Specialty Infusion. The role involves providing advanced clinical care to patients in their homes, managing caseloads, developing and following care plans, and collaborating with physicians.
Job Responsibility:
Assumes primary responsibility for caseloads consisting of patients in specialty area
Conducts initial assessment to obtain thorough physical, social and environmental data. Establishes and follows a care plan, identifies problems, and evaluates outcomes within a certain time frame
Communicates with physicians to confirm, correlate, and update patients' plan of care. Provides status update to physician on a regular basis
Provides on-going assessment of patient's response to services and teaching and applies interventions as appropriate. Updates and revises care plan as needed
Facilitates the discharge planning process according to standards and utilizes resources to assure continuity of care after discharge
Make independent clinical decisions to optimize goals of therapy and patient outcomes
Maintains and updates accurate clinical and patient records according to agency, state, and federal guidelines
Communicates problems to physicians and clearly states plans, actions, and goals so that changes are reflected as they occur
Participates in Continuous Quality Improvement activities and inservices as required
Follows published agency policies and procedures and regularly monitor changes or additions to them
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures
Requirements:
Registered Nurse license issued by the state in which the team member practices
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
A valid drivers license issued by the Division of Motor Vehicles
Associate's Degree in Nursing
Typically requires 1 year of experience in acute clinical nursing
Proficient in advanced clinical skills with the ability to work independently and make sound judgements
Ability to assist with the analysis of data and the development of appropriate clinical tools required in management of the patient in a complex nursing area
Ability to concentrate on minute detail and handle multiple tasks with frequents interruptions
Must be able to sit, stand, walk and climb stairs for prolonged periods of time. Must also be able to move about in confined spaces, including bending, twisting, squatting and occasionally reaching one or both arms over head
Must be able to frequently move, lift and support up to 50 pounds
Must have manual dexterity required to use and operate a variety of medical equipment and supplies, writing implements and office equipment
Must have functional speech and hearing to communicate with people in person or over the telephone
Must have functional vision to read gauges and measuring devices and to interpret written instructions
