Graduate of an accredited nursing program
must hold an active RN license in the state served
At least 1 year of nursing experience (Home Health preferred)
Strong critical thinking, clinical expertise, case management, and a passion for patient care
CPR/BLS certification

Job Responsibility

Deliver compassionate, high-quality nursing care tailored to each patient’s needs
Develop and adjust individualized care plans to ensure optimal patient outcomes
Collaborate with physicians, therapists, and other healthcare professionals
Supervise your own caseload, nurturing the growth and development of LPNs, LVNs, and Home Health Aides, to facilitate and cultivate a team of caregivers
Provide education and support to patients and their families
Maintain thorough documentation to meet regulatory and operational standards

Job Responsibility

Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care
Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members
Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care
Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.)
Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG
Performs other duties/projects as assigned
