Requirements

Graduation from an Occupational Therapist Assistant program from an accredited university/program
Current, unrestricted state license as an Occupational Therapy Assistant in the state of practice
Valid CPR, Drivers License and Proof of Automobile Insurance
HCHB and Home Health experience preferred

Job Responsibility

Provides direct occupational therapy according to directions of occupational therapist and in accordance with the physician’s plan of care
Communicates with and regularly updates the patient and/or caregiver regarding the plan of care
Treats clients to relieve pain and/or develop or restore function to maintain maximum performance
Provides skilled teaching to the patients, family members, and caregivers regarding the safe use of assistive devices and durable medical equipment
Educates patients and caregivers in carry over regarding issues concerning rehabilitation and adaptation techniques needed for recovery
Collaborates with other service disciplines to discuss the patient plan of care
Plans for client’s discharge, prepares discharge instructions, and communicates discharge plan with patients, family members, and caregivers
Records clinical and progress notes at the time of the visit to ensure continuity of qualitative patient care on a timely basis
Completes daily report and any required documentation in a timely fashion to ensure agency operating schedules
Maintains consistent attendance

A person who meets the requirements for certification as an Occupational Therapy Assistant established by the American Occupational Therapy Association (AOTO)
Certification is maintained by the AOTA
Currently licensed in the state(s) in which practicing
Two (2) years of experience, preferred.

Job Responsibility

Understands and adheres to established Agency policies and procedures
Improves or minimizes residual physical disabilities of the patient
Returns the individual to optimum and productive level within the patient’s capabilities
Participates with all other health care personnel in patient care planning
Performs all skilled procedures as ordered by physician and according to the plan of care established by the OT
Consults with OT regarding change in treatment
Instructs patients and family members in home programs and fine motor movement exercises
Participates in in-service education and presents in-service programs as assigned
Participates in performance improvement activities as assigned
Attends all patient care conferences as scheduled

Requirements

Occupational Therapist license issued by the state of Wisconsin
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
A valid drivers license issued by the Division of Motor Vehicles
Bachelor's Degree in Occupational Therapy
Typically requires 3 years of experience in occupational therapy within an acute hospital, home health care, or other relevant environment
Demonstrated proficiency in clinical skills with the ability to work independently and make sound judgments
Good human relations, organization, problem solving, and decision-making skills
Basic proficiency in the use of Microsoft Office (Excel, PowerPoint and Word) or similar products
Must be able to obtain transportation to attend to the daily schedule of home visits and other business in a timely manner
Must be capable of the manual dexterity required to use and operate a variety of medical equipment and supplies, writing implements and office equipment

Job Responsibility

Evaluates the patient's physical, perceptual, cognitive, social, and vocational abilities and their impact on the patient's activities of daily living
Conducts an occupational therapy evaluation of each patient and establishes a plan of care and treatment
Maintains and updates accurate clinical and patient records according to state and federal guidelines
Plans and implements individual treatment to re-train and teach compensatory techniques for residual disability in activities of daily living, adapted homemaking techniques, bathroom transfers, functional application of mobility skills and, when appropriate, training in adapted work methods
Discusses caseload issues and changes in case census with management
Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur
Communicates with physicians and other team members to confirm and update patient plan of treatment and provides physician with status report on a regular basis
Manages assigned caseload, including scheduling and informing site leadership of coverage needs and ability to provide coverage
Manages the patient's medical condition including medications
Teaches appropriate treatment procedures and supportive activities to patients and caregivers, and develops plan for obtaining adaptive devices as needed
