Job Responsibility:

  • Review documentation and provide coding and sequencing as it relates to the patient s medical condition and the reason for home health using current ICD-10 CM coding and guidelines
  • Prospectively reviews all OASIS assessments and 485 s to ensure appropriateness, completeness and compliance with federal and state regulations and organization policy
  • Observes and reports unusual patterns in data collection and/or lack of adequate documentation for code assignments
  • Perform physician query as needed to clarify documentation in the health record and achieve accurate code assignments
  • Consults with appropriate clinical staff to clarify and data integrity and works to ensure appropriate corrections are made per organization policy
  • Maintains daily productivity and turnaround times as outlined in department s performance improvement plan
  • Performs other related duties as requested by the Home Health Regional Director of Quality Assurance

Requirements:

  • Certification in Home Health Coding (BSHHC or HCS-D) highly preferred
  • Certification in OASIS (COS-C or HCS-O) highly preferred
  • Working knowledge of CMS Condition of Participation in Home Health
  • Working knowledge of federal and state home health licensure regulations
  • At least one year of experience working in a home health setting
  • Sound computer skills and adaptive to home health documentation software
  • Knowledge of medical terminology, anatomy and physiology, compliance and reimbursement guidelines are required
  • Detail oriented with critical thinking and strong analytical skills
  • Must be self-motivated, responsive and committed to communicating with management
  • Ability to demonstrate flexibility in response to changes in work volume and work schedules
  • Excellent interpersonal skills including active listening, conflict resolution and team building
  • Strong people skills and supervisory capacity will prove tremendously helpful for success

  • Adheres to the organization and departmental guidelines, policies and protocols
  • Reviews all clinician documentation to support assigned codes in the health information record so that all significant diagnoses and procedures may be captured for reimbursement and data purposes
  • Conduct independent research to promote knowledge of coding guidelines, regulatory policies and trends
  • Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association and adheres to official coding guidelines
  • Practices ethical judgment in assigning and sequencing codes for proper insurance reimbursement
  • Maintains the confidentiality of patient records
  • Reports any perceived non-compliant practices to the coding leader or compliance officer
  • Meets then exceeds departmental quality and productivity standards
