Providing holistic support that enables individuals to be discharged from hospital feeling supported, confident, and empowered. This includes identifying and coordinating the practical, emotional, and social support needed to reduce the risk of hospital readmission or future admissions. All support is delivered within a strengths‑based approach that promotes wellbeing, independence, confidence, and overall quality of life.
Job Responsibility:
Ensure day-to-day delivery of the service in line with contractual and organisational requirements
Be first point of contact for client referrals, processing within timescales, assessing needs, identifying support, escalating safeguarding concerns
Work with statutory services to agree contributions, liaise with agencies and clients
Provide low-level support for clients up to 6 weeks via direct home visits or volunteer oversight
Work with other Coordinator to ensure resource use and cover
Establish and maintain working relationships with internal/external teams
Recruit, induct and supervise volunteers
Promote service and volunteering opportunities
Collate monitoring data and prepare reports
Ensure data processed in line with GDPR and procedures
Undertake risk assessments
Assist in delivering within budget
Comply with policies and procedures
Carry out other duties as required
Requirements:
Desire to provide support and help people, some of whom may be in difficult circumstances
Patience and empathy for the needs and expectations of clients and referring agencies
Very good listening, verbal and written communication skills
Strong interpersonal skills and ability to build effective working relationships with people from a variety of personal and professional backgrounds
Experience of working in a health or social care setting that is supporting people
Ability to undertake needs assessments and taking the right actions for implementing an individual’s practical, emotional and social support
Ability to identify, manage and appropriately escalate safeguarding concerns
Ability to work on own initiative and as part of a team
Ability to enthuse people to volunteer for the service and to provide the effective training and support
Good organisational skills and ability to handle conflicting work pressures
Confident and self-sufficient IT user, particularly Microsoft Word, CRM, Excel, Outlook and Teams including for monitoring and reporting purposes
Non-judgemental disposition and commitment to principles of confidentiality and safeguarding
Ability and willingness to regularly travel across the work area, including rural and remote communities, and to attend external meetings with associated organisations, using your own reliable transport

Experience of working in service that seeks the reablement of people following a hospital stay or other health related changes in their lives
