Under the direction of the Clinical Manager, evaluates, plans, and administers medically prescribed physical therapy treatments to patients in their home
Performs a comprehensive physical, psycho-social, and safety assessment of assigned patients
Completes thorough documentation of Initial Assessment with results recorded in measurable terms and a Plan of Care is established
Plans for and provides skilled physical therapy treatment in the home reassessing the patient’s status on each visit
Coordinates patient care with other clinical team members ensuring optimal patient care and communication
Agency Recertifications are completed within 5 days prior to the end of Certification period
Discharge planning occurs throughout the episode of care with documentation reflecting pt and Care Plan Partner have been notified and prepared for discharge
Demonstrates clinical proficiency and competency in the agency/program automated clinical record
Fosters a “Culture of Safety” through personal ownership and commitment to a safe environment
Maintains up to date schedule in computerized system, identifying availability for new patients
Maintains and enhances individual knowledge as well as promoting professional standards in physical therapy
Participates in continued learning and possess a willingness and ability to learn and utilize new technology and procedures
Requirements:
BS degree in Physical Therapy preferred
1-2 years of clinical experience with a varied patient population
