Provides patient care, under the supervision of a Case Manager: Nurse or therapist. Provides personal care/companionship as directed by plan of care. Provides light housekeeping/ light meal prep tasks as directed by plan of care. Effectively communicates both verbally and in writing. Initiates and maintains positive relationships with patients/customers and co-workers. Observes and reports patient status changes including skin and psychosocial. Takes responsibility for self-development and supports a learning environment. Displays commitment to the mission of UHHS/UHHCS and its values. Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Part time 20 hours per week. 100% travel to the patients homes to provide care. The territories are Ashland County. Monday-Friday.

Requirements:
High School Equivalent / GED required
Completes required continuing education requirements to maintain certification required
Certified Nursing Assistant (CNA) or equivalent certification required
Basic Life Support (BLS) Required within 30 Days
OH Driver’s License (Valid) with car insurance coverage required
Successful completion of agency written test and competency evaluation of skills in the field Required within 30 Days
Nice to have:
1+ years Home care experience preferred
