Home Care Assistant

• Provides patient care, under the supervision of a Case Manager: Nurse or therapist
• Provides personal care/companionship as directed by plan of care
• Provides light housekeeping/ light meal prep tasks as directed by plan of care
• Effectively communicates both verbally and in writing
• Initiates and maintains positive relationships with patients/customers and co-workers
• Observes and reports patient status changes including skin and psychosocial
• Takes responsibility for self-development and supports a learning environment
• Displays commitment to the mission of UHHS/UHHCS and its values
• Performs other duties as assigned
• Complies with all policies and standards
• Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients
• 100% travel to the patients homes to provide care

• High School Equivalent / GED required
• Completes required continuing education requirements to maintain certification required
• Certified Nursing Assistant (CNA) or equivalent certification required
• Basic Life Support (BLS) Required within 30 Days
• OH Driver’s License (Valid) with car insurance coverage required
• Successful completion of agency written test and competency evaluation of skills in the field Required within 30 Days

1+ years Home care experience preferred
