Helpline Care Coordinator

• Provide follow-up and care coordination for clients needing additional support and advocacy
• Assist clients with system navigation through intake, screening, assessment, referral, and service linkage via phone and email
• Provide crisis counseling and suicide intervention support when needed
• Complete safety/risk screenings and person-centered assessments
• Coordinate referrals and warm transfers to appropriate community resources
• Advocate for clients and maintain accurate case documentation and reporting
• Support maintenance of the Community Resource Directory and Data Resource initiatives
• Participate in trainings, supervision, and team meetings
• Represent the Contact Center at outreach events and community boards as needed

• Bachelor’s degree in Social Work, Psychology, or related field
• Minimum 2 year of experience in crisis counseling or clinical work
• Experience providing information and referral services
• Experience working with veteran populations preferred
• Strong interpersonal and communication skills
• Proficient with Microsoft Office applications
• Fully bilingual English/Spanish required
