A mission-driven organization is seeking a compassionate and professional Helpline Care Coordinator to provide support, advocacy, and care coordination services for individuals in need. This role is ideal for someone passionate about helping others, crisis intervention, and community resources.
Job Responsibility:
Provide follow-up and care coordination for clients needing additional support and advocacy
Assist clients with system navigation through intake, screening, assessment, referral, and service linkage via phone and email
Provide crisis counseling and suicide intervention support when needed
Complete safety/risk screenings and person-centered assessments
Coordinate referrals and warm transfers to appropriate community resources
Advocate for clients and maintain accurate case documentation and reporting
Support maintenance of the Community Resource Directory and Data Resource initiatives
Participate in trainings, supervision, and team meetings
Represent the Contact Center at outreach events and community boards as needed
Requirements:
Bachelor’s degree in Social Work, Psychology, or related field
Minimum 2 year of experience in crisis counseling or clinical work
Experience providing information and referral services
Experience working with veteran populations preferred
