The Heart Failure Nurse Practitioner/Candidate aims to optimise patient outcomes during the acute phase of illness in ED and inpatient setting and during the recovery/transitional period in the outpatient clinic and via telehealth. This role will also drive evidence based care through the Virtual Hospital Pilot, including supporting virtual ward activities and regional partnerships for patients with Heart Failure. The Heart Failure Nurse Practitioner (NP)/Candidate (NPc) is directly involved in the provision of expert patient-centred clinical care and is recognised as a senior clinical nurse in their area of specialty. This extends to the care of family, carers, and other health professionals.
Job Responsibility
Providing high quality care to patients with Heart Failure in both the inpatient, outpatient, and virtual care settings
Work collaboratively with clinicians within the admitting unit and GPs, supported by the Director of Heart Failure Services & Cardiology to improve systems of care.
Requirements
Extensive experience in the management of heart failure in the acute setting both inpatient and outpatient
Have or working towards Nursing & Midwifery Board of Australia endorsement as a Nurse Practitioner
Post graduate certificate/diploma in critical care nursing.
