Level 1 registered Nurse or registered Paramedic
minimum of three years post registration experience
heart failure experience
palliative care or oncology experience
ability to work autonomously and manage a caseload

Job Responsibility

Delivering high-quality specialist heart failure and palliative/end-of-life care for patients
working across eastern Hertfordshire in partnership with acute hospital trusts, community teams, and primary care services
working closely with Community Specialist team, Hospice at Home team and In Patient Unit
providing cover to these services if required
