Job Description

We are looking for an experienced Heart Failure Clinical Specialist (Nurse/Paramedic) to join our community team. This is an exciting opportunity to play a key role in delivering high-quality specialist heart failure and palliative/end-of-life care for patients

Job Responsibility

  • Delivering high-quality specialist heart failure and palliative/end-of-life care for patients
  • working across eastern Hertfordshire in partnership with acute hospital trusts, community teams, and primary care services
  • working closely with Community Specialist team, Hospice at Home team and In Patient Unit
  • providing cover to these services if required

Requirements

  • Level 1 registered Nurse or registered Paramedic
  • minimum of three years post registration experience
  • heart failure experience
  • palliative care or oncology experience
  • ability to work autonomously and manage a caseload

Nice to have

  • palliative care or oncology qualification
  • non-medical prescribing qualification
