We have a rewarding opportunity for a dedicated Healthcare Practitioner - All Age Continuing to join our team on a full time, permanent basis, working as part of a collaborative and compassionate service. You will play a key role as part of a small specialist team providing advice and support across all our social care teams in adults and children.
Job Responsibility:
Support the All Age Continuing Healthcare Manager to work across all social care teams, providing technical advice in relation to the National Framework for NHS CHC and FNC (2022) and its interface with other legislation
Represent social care at multi-disciplinary CHC assessments, recording outcomes and updating records
In absence of Team Manager, liaise with Team Managers to deliver progress reports and represent the service at meetings
Support the CHC Team in the delivery of quarterly CHC training to council and NHS staff
Build effective working relationships with key stakeholders within the council and external partners
Requirements:
Practical knowledge of the Care Act 2014 and its complex interface with NHS Continuing Healthcare eligibility
Strong analytical skills to gather and use information from a range of health and social care reports and assessments
Ability to professionally challenge when required
Background in social care or health
