Job Description

The Health Unit Coordinator performs clerical duties and coordinates communication of a unit to help patient care activities.

Job Responsibility

  • Performs clerical duties to support patient care activities
  • Initiates, assembles, and maintains patient charts following procedures
  • Files reports and test results
  • Processes ordered tests, medications, procedures, and appointments
  • Processes patient admissions, transfers, and discharges
  • Informs team members of reports, orders, and other information
  • Answers telephones and manages the patient call system
  • Serves as an information resource to patients, staff, and the public
  • Consistently interacts appropriately with all ages
  • Completes and maintains unit and patient records, files, forms, and documents
  • Maintains an organized work area of records, files, forms, and reference documents
  • Checks, updates, and maintains facility boards, pneumatic tube system, and telemetry box reconciliation according to departmental policy, procedures, and guidelines
  • Prepares reports, purchase orders, projects, memos, and letters using computer programs
  • Greets visitors and helps them as needed
  • Answers telephones and contacts nursing team and medical staff using pagers, beepers, and cell phones
  • Tracks call light response time for the unit
  • Assigns phone and location devices to appropriate staff
  • Performs work area duties as directed by the nurse or physician
  • Runs errands, picks up supplies, and transports patients
  • Maintains par levels of inventory, including requisitions/forms, and stocks work area supplies and equipment
  • Maintains a clean, safe, and functional work area
  • Coordinates transportation for patients and patient items as delegated

Requirements

  • H.S. Diploma/GED Equivalent
  • Less than 1 Year of Experience
  • Excellent bedside manner and ability to make patients feel at ease
  • Ability to take instruction from a doctor
  • Familiarity with medical charts and records
  • High attention to detail
  • Knowledge of basic computer and keyboard skills
  • Completion of Health Unit Coordinator Training Program preferred

  • Provides physical and psychosocial support, patient education, wound care, management of hypertension, phlebotomy and general nursing care
  • Develops a nursing care plan for kidney transplant recipients that include assisting patients with independent management of prescribed medical regimen
  • Monitors patients to rule out rejection, and documents patient lab results and physical findings, and collaborates with surgeons and nephrologist to provide treatment options
  • Reviews and interprets patient test results in collaboration with surgeons and nephrologists
  • Coordinates surgical and diagnostic interventions prescribed by the surgeon and/or nephrologists’ transplant process
  • Contacts patients (timeliness is essential) at end of clinic visit to review plan of care and make any necessary adjustments in anti-rejection medications
  • Assists hospital nursing staff in the post-operative care of the kidney transplant.regarding the patients through care coordination and oversight
  • Assists in the coordination of patient care and the monitoring of nursing care of the kidney transplant recipient through active involvement in treatment planning
