Job Description

As a One Medical Flex Health Navigator supporting our Senior patients (age 65+) on Medicare, you are focused on delivering exceptional care coordination and team assistance. This role is primarily serving patients with complex care needs who benefit from coordination, advocacy, and navigation through both virtual and in-person interactions. As a Health Navigator, you will serve as a crucial link between patients, our providers, and the broader healthcare system, ensuring seamless care delivery and improved health outcomes. This position offers the opportunity to make a meaningful impact on patient care while working in a dynamic, innovative healthcare environment committed to transforming the healthcare experience and outcomes for seniors. Our Flex Health Navigators lend support multiple offices in a MSA.

Job Responsibility

  • Manage comprehensive care coordination for senior patients, including specialist referrals, appointment scheduling, and follow-up with high-risk or disengaged patients
  • Conduct pre-visit screenings and post-visit check-ins to identify care priorities, gaps, and ensure continuity across internal and external care teams
  • Support preventive care by monitoring patient panels, conducting cognitive screenings, and resolving care gaps through proactive outreach
  • Provide patient education on chronic disease management using motivational interviewing and action-planning techniques
  • Contribute to daily clinic operations, including phone support, front desk assistance, and maintaining clinical spaces

Requirements

  • At least 1 year of experience in a high touch patient-facing healthcare role
  • Strong EMR system proficiency and comfort using healthcare technology platforms
  • Proficient in Google Workspace (Sheets, Docs, Slides) and adept at navigating multiple software systems simultaneously
  • Excellent written and verbal communication skills with demonstrated ability to collaborate effectively on diverse, multi-disciplinary teams
  • Self-motivated with proven ability to work independently, adapt to change, and proactively address challenges under pressure
