The Department of Healthcare and Family Services is seeking to fill a Health Facilities Surveillance Nurse for the Bureau of Long-Term Care. The position will be conducting inspections and providing consultation to Long Term Care providers. The ideal candidate will work closely with long term care settings regarding certification standards, clinical practice standards, and long-term care issues. The ideal candidate will also monitor the implementation of the Medicaid Waiver program and assist with certification and monitoring of the health and safety of residents in the Supportive Living Program settings.
Job Responsibility:
Under general supervision, monitors the operations of the agency administered Medicaid Waiver for the Supportive Living Program (SLP)
Conducts annual reviews of the agency administered Medicaid Waiver for the SLP to determine compliance with federal and state regulations and requirements
Conducts investigations of Complaints and Incident Reports received by the Department related to the residents in the Medicaid Waiver SLP setting to determine compliance with federal and state regulations and requirements
Conducts initial and annual certification inspections of the Medicaid Waiver SLP settings to determine compliance with federal and state regulations and requirements
Provides technical assistance
Conducts Minimum Data Set (MDS) audits, periodic bed census surveys and other reviews as determined by the Department
Attends staff meetings to remain current on unit activities and processes
Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above
Requirements:
Requires (a) graduation from an approved nursing education program with an Associate degree or a Diploma in Nursing and three (3) years of professional experience
(b) a Bachelor's degree in Nursing and two (2) years of professional nursing experience
(c) a Master's degree in Nursing
Requires licensure as a Registered Nurse in the State of Illinois
Requires completion of a background check and self-disclosure of criminal history
Requires ability to travel in the performance of duties, with overnight stays as appropriate
Requires appropriate, valid driver’s license
Requires the ability to utilize and maintain state issued equipment such as a laptop
Nice to have:
Two (2) years of experience interpreting federal and state regulations to ensure compliance of long-term care providers
Two (2) years of experience overseeing nursing principles and practices
Two (2) years of experience working with a Medicaid Home and Community Based Services (HCBS) Waiver Program
Two (2) years of experience regarding the process of licensure and certification requirements of long term care providers
Two (2) years of experience with case management and the coordination of services for individuals in a long term care or assisted living facility
Two (2) years of experience working with the survey process used to measure and monitor conditions of long-term care settings and residents to assess compliance with federal and state requirements
