Job Description

Health Coach position at Advocate Health, collaborating with patients to actively engage a population of medically vulnerable or chronically ill patients in their care, assisting them through the process of actively working towards better health by providing support, encouragement, and education.

Job Responsibility

  • Collaborates with patients/teammates to engage medically vulnerable or chronically ill patients in their care
  • Communicates with patients/teammates in person, in groups, by telephone and electronically
  • Implements skills including coaching, motivational interviewing, behavior modification, goal setting, barrier assessment and problem solving
  • Advocates and facilitates referrals to services and resources
  • Provides customized, evidence-based patient/teammate education
  • Encourages patient/teammate to participate in shared decision making
  • Communicates care plans with the primary care team and other specialists
  • Assists with coordination of care across the care continuum

Requirements

  • High School diploma or equivalent required
  • Bachelor's degree in health discipline preferred
  • Health Coach Certification required (or attained within one to two years of employment)
  • Physical ability to walk, stand, sit, lift, reach, bend, push and pull
  • Ability to lift 50 pounds
  • Intact sense of sight, hearing, smell, touch and finger dexterity
  • Ability to move about the medical center and travel to various clinical and community locations
