Job Description

We are looking for a passionate and skilled Exercise Referral Specialist who is dedicated to making a positive impact on people’s lives. This role requires a deep commitment to helping individuals with medical conditions improve their health, confidence, and overall wellbeing through tailored physical activity programmes. Beyond delivering personalised exercise support, you will play a key role in developing, coordinating, and delivering funded programmes that promote healthier lifestyles within the community.

Job Responsibility

  • Designing individualised exercise programs based on the referral information and initial assessments
  • Ensure that exercise plans are safe, effective, and aligned with the clients’ health conditions and goals
  • Conduct initial consultations and health assessments for referred clients
  • Provide one-on-one instruction and guidance on the correct use of fitness equipment and proper exercise techniques
  • Monitor and record clients’ progress, providing regular feedback and adjustments to their exercise plans
  • Offer ongoing support, motivation, and education to encourage adherence to exercise programs
  • Liaise with healthcare professionals to ensure a comprehensive approach to clients’ health and wellbeing
  • Work with the Health and Wellbeing Team to integrate exercise referral schemes within the broader health promotion activities at the centre
  • Participate in team meetings and contribute to the continuous improvement of the exercise referral service
  • Deliver educational workshops and group sessions on various health and fitness topics
  • Stay updated with the latest research and best practices in exercise referral and related health areas
  • Attend professional development courses as required to maintain relevant qualifications and skills
  • Prepare reports on program outcomes and client progress for internal use and external stakeholders as needed
  • Manage scheduling of client appointments and follow-up sessions efficiently

Requirements

  • Level 2 Fitness Instructor qualification
  • Level 3 Exercise Referral Diploma or equivalent
  • Experience working with individuals with various health conditions (e.g., obesity, diabetes, hypertension)
  • Knowledge of health promotion and public health principles

Nice to have

Certifications in specific health conditions or exercise modalities (e.g., cardiac rehabilitation, pulmonary rehabilitation, falls prevention)
