The HACC-PYP Coordinator is responsible for coordinating services under the Home and Community Care Program for Younger People (HACC-PYP), which supports Victorians under 65 (or under 50 for Aboriginal and Torres Strait Islander people) who are at risk of losing independence due to disability, chronic illness, mental health or other conditions. This role ensures participants receive timely, person-centred, and goal-oriented support that promotes independence, wellbeing and community participation. The HACC-PYP Coordinator upholds the principles of wellness, reablement and restorative care, while meeting program guidelines and compliance requirements.

Requirements:
Certificate IV or Diploma in Community Services, Disability, Social Work or a related field
Current Drivers Licence
