GP Telephone & Video Consultations

We are looking for Locum GPs who can cover telephone triage, along with video consultations, some home visits if required. Duty: (NO On call). Reviewing patients referred by the acute trust post discharge and referrals from practice GPs for patients who are struggling at home. This will support older people with frailty who have moderate/severe COVID symptoms and are not for hospital admission, providing supportive and palliative care at home. You will be working with other GPs, community matrons, pharmacists, mental health professionals, social workers and wellbeing coordinators and you will have access to geriatrician consultant support and access to palliative care consultant support.
